At OFSS it is our aim to always provide an efficient and effective service.

However, if you would like to provide feedback or make a complaint about the service provided, the feedback & complaints form can be accessed via the button below.

How can you lodge a complaint?

You can lodge your complaint verbally or in writing.

The Complaint Form will guide you with the detail that is required to lodge a complaint.

Once you have gathered your information you can:

When your complaint has been received, we will:

  • Process: We will ensure your complaint is acknowledged within 14 days

  • Investigate: Including keeping you informed of the process

  • Resolve: In a prompt manner, usually not more than 28 days, giving you reasons for our decisions

No one will be penalised for making a complaint of any kind.
